Queen of Equestria[1] is the highest title in the post-Reign of the Royal Sisters Equestria. So far, the only known queen has been Twilight Sparkle, who had previously ruled as the Princess of Friendship. However, the role was functionally filled by Princess Celestia at least during the period of Princess Luna’s confinement on the Moon. Indeed, this may have been the reason for Lord Gestal addressing her as Queen, though her own subjects used the title of Princess; the latter title was also applied by Celestia to Twilight during the latter’s coronation.
Although Twilight was told she would inherent the title before the formation of the Legion of Doom, she ultimately didn't inherent the title until the violent Apocalypse, in which the aforementioned Legion very nearly took over all of Equestria. After her final coronation, Twilight's first known act as queen was to organize several expeditions, including one into Farasi, the country of the Zebras.
